2012 - 2013 School Year
Please note: If you do not enroll for the year in the fall semester, you will not be able to enroll later during the 2012-2013 school year without experiencing a “life change”. This means that the loss of coverage is no fault of your own or the policyholders (i.e. you get married or become too old to be on a family plan). You will be eligible to enroll within thirty days of this “life change”. After thirty days, you will no
longer be eligible.

HARD WAIVER STUDENT ENROLLMENT INSTRUCTIONS
If you are an……
undergraduate student taking 6 or more credit hours or a graduate student taking 5 or more credit hours, you are required to purchase this Plan, unless proof of comparable coverage is furnished. All international F-1 Visa students are required to purchase this plan. F-1 Visa students are automatically enrolled in this insurance plan at registration and the premium for coverage is added to their tuition billing.

- Summer Enrollment Deadline: Summer is Voluntary (For new incoming students only. Please see Life Change & Dependent Enrollment Instructions below.)
If you have missed the hard waiver enrollment deadline, but have not previously waived out of the insurance, you will be automatically enrolled in the insurance by your school, after the waiver deadline.
If you have previously waived out of the insurance and would like to enroll, or if your non-school sponsored coverage is being terminated and you are now looking to enroll in the

DEPENDENTS
Eligible students who do enroll may also insure their Dependents. Eligible Dependents are the spouse and unmarried children under 26 years of age who are not self-supporting. To enroll, please see Life Change & Dependent Enrollment Instructions below.

To enroll: download an enrollment form and submit by mail using a check or money order. The mailing address is listed on the back of the Enrollment Forms.
The effective date of your coverage will be the date we received the enrollment form with full payment and any necessary information in our office.
