We suggest you review the brochure for your rates, effective dates and optional coverages before starting the enrollment process. The plan’s benefits can be found in the brochure within the Schedule of Benefits section with the exclusions towards the back.

2012 - 2013 School Year – Fall Term

 HARD WAIVER STUDENT ENROLLMENT INSTRUCTIONS

Domestic and International (Full Time Students)
All full-time undergraduate students (12 or more credit hours) and all full-time graduate students (9 or more credits) entering in the fall term will automatically be assessed an insurance charge and enrolled in this health insurance plan unless proof of comparable coverage is furnished. The premium covers the fall, winter, spring, and summer terms.

International Students (Part-Time Students)
All part-time undergraduate (less than 12 credit hours) and graduate (less than 9 credit hours) F1 or J1 international students entering in the fall term will automatically be assessed an insurance charge and enrolled in this health insurance plan  unless proof of comparable coverage is furnished. The premium covers the fall, winter, spring, and summer terms. 
  • Fall Enrollment Deadline: 9/16/12
  • Spring/Summer Enrollment Deadline: 2/1/13
  • Summer Enrollment Deadline: 5/31/13 Summer is voluntary (If you are a new incoming student for the Summer Session, you must use the enrollment form below)
If you have previously waived out of the insurance and would like to enroll please download the enrollment form listed below and submit by mail using a check or money order. 
 
PLEASE NOTE, Hard Waiver Students: The Optional Major Medical Coverage cannot be purchased through the Enroll/Waive site. For Hard Waiver Students to enroll in the Optional Major Medical coverage, you must fill out an enrollment form and mail it in with your payment. The enrollment form with payment must be received in our office within two weeks after enrolling in the basic coverage. If received after the two week grace period, you will not be eligible to enroll in the Optional Major Medical and your enrollment form and payment will be returned to you.

 


 

Major Medical

 Major Medical is an additional optional benefit you may choose to purchase, in addition to the basic policy.

Hard-Waiver Students, Voluntary Students, and Dependents are eligible to purchase the Major Medical benefit for an additional premium.
Hard-Waiver Students have 2 weeks from the date of their enrollment in the basic program to enroll themselves in the Major Medical benefit.
Voluntary Students and all Dependents must enroll in Major Medical at the time of their initial enrollment.

Voluntary Students

 Part-Time Students (Non-International Students Only)
All part-time non-international undergraduate (less than 12 credit hours) and all part-time graduate students (less than 9 credit hours) are eligible to enroll in this plan by applying directly to the insurance company. The premium will not appear on a student’s term bill. You will have to enroll online or with an enrollment form. Please see Voluntary Students & Dependent Enrollment Instructions below.

 
PLEASE NOTE, VOLUNTARY STUDENTS: The Optional Major Medical Coverage must be purchased simultaneously and in conjunction with the purchase of Basic coverage at the time of initial enrollment in the plan for you and your dependents.

 

Dependents

Part-time and full-time students enrolled in this insurance Plan may purchase dependent coverage directly through the insurance company. Eligible dependents are the spouse (husband, wife, or Civil Union partner) or Domestic Partner and dependent children up to 30 years of age. The premium will not appear on a student’s term bill. You will have to enroll online or with an enrollment form. Please see Voluntary Students & Dependent Enrollment Instructions below.

Enrollment Instructions For Part Time Students, Dependents, and Major Medical Coverage

 

IMPORTANT INFORMATION: Open Enrollment Periods for all Dependents and Part-Time Students: If you have eligible Dependents in the fall or, are a Part-Time student in the fall semester and eligible to purchase coverage and you choose not to enroll for coverage before the Fall Enrollment Deadline of *September 14, 2012, your Dependents or you will not be eligible to enroll again until the start of next fall unless you experience a “Life Status” change during the year. A life status change includes marriage, divorce, birth of a child or loss of coverage through no fault of your own (i.e. aging of your parents’ coverage). If your Dependents or you, experience a “Life Status” change, you must submit proof of the event and enroll within 30 Days of the event, otherwise you will no longer be eligible to enroll for the remainder of the policy year.

 
*For new Dependents or new Part-Time students in the spring semester, your open enrollment deadline is February 1, 2013.
 
*For new Dependents or new Part-Time students in the summer semester, your open enrollment deadline is May 31, 2013.
 
Open Enrollment Deadlines
  •  Annual/Fall Enrollment Deadline: 9/14/12
  • Spring/Summer Enrollment Deadline: 2/1/13 (For new incoming students & insured fall students renewing coverage)
  • Summer Enrollment Deadline: 5/31/13 Summer is voluntary (For Part-time students starting in a program in the summer and accepted into a program in the Fall of 2013)

Health insurance will not automatically be charged to any undergraduate or graduate student for the summer term. Coverage may be purchased on a voluntary basis by enrolling online or submitting an enrollment form.  The premium covers the summer term only.

Travel Assistance is included in the Ramapo College of New Jersey Student Injury and Sickness Plan.  If you did not enroll in your school's plan, you may still enroll only for the Travel Assistance Program.
