2012 - 2013 School Year

Please note: Open Enrollment Periods for all Students: If you are a Full Time student in the fall semester and eligible to purchase coverage and you choose not to enroll for coverage before the fall Enrollment Deadline of *November 22, 2012, you will not be eligible to enroll again until the start of next fall, unless you experience a “Life Status” change during the year. A life status change includes marriage, divorce, birth of a child or loss of coverage through no fault of your own (i.e. aging of your parents coverage). If you experience a “Life Status” change, you must submit proof of the event and enroll within 30 Days of the event, otherwise you will no longer be eligible to enroll for the remainder of the policy year.

*For new Full-Time students in the Fall semester, your open enrollment deadline is November 22, 2012.
*For new Full-Time students in the Winter semester, your open enrollment deadline is February 23, 2013.
*For new Full-Time students in the Spring semester, your open enrollment deadline is May 23, 2013.

VOLUNTARY STUDENTS
If you are a….
Full Time Student enrolled in and actively pursuing a full time program and students engaged in internship programs where credit is needed to graduate or complete course of study, you are eligible to purchase this insurance on a voluntary basis. Please see Enrollment Instructions below.
 
If your non-school sponsored coverage is being terminated and you are now looking to enroll in the Student Injury & Sickness plan, 
 
DEPENDENTS
Dependent coverage is not available.
